			<content:encoded><![CDATA[<p>Complimentary and alternative medicine is being used by 40 percent of Americans and is a $21 billion industry, paid for almost exclusively with out-of-pocket funds. <span id="more-394"></span></p>
<p>Alternative care is apparently being used to augment traditional care, as the Journal of the American Medical Association (Nov. 1998) reported that the number of visits to complimentary and alternative healthcare practitioners increased from 427 million in 1990 to 629 million in 1997, whereas visits to primary care physicians stayed essentially the same (388 million in 1990 and 386 million in 1997). This finding indicates that consumers are taking responsibility for their health and are making a concerted effort to stay healthy, a concept consistent with the mission of health clubs.</p>
<p><a href="http://herbaldrugstore.org/articles/herbal-remedies-are-medicine.html">Alternative healthcare that is offered by health clubs includes Pilates, nutritional counseling, yoga, aquatic therapy, tai chi, personal training and massage.</a></p>
<p> Additionally, many health clubs also offer chiropractic services, acupuncture, rolfing, physical therapy, biofeedback and other alternative services to meet the demands of their members.</p>

&#8220;Another message we keep getting from the media is that if you find it early there is 97 percent survival rate. That message is intended not to frighten women. But the reality is that breast cancer is far more complex than that,&#8221; Mayer emphasized.</p>
<p style="text-align: justify;"><span id="more-390"></span>A small tumor, for example, might be composed of aggressive cells, some of which may have strayed to distant sites by the time the tumor is found. Mindful of this possibility, she believes that it is important for women to try to grasp the dynamics of breast cancer.</p>
<p style="text-align: justify;">Mayer, who recently won an advocacy award from the National Breast Cancer Coalition, distilled many of her thoughts on breast cancer, its diagnosis, treatment and recurrence, in two books. The first was &#8220;Examining Myself: One Woman&#8217;s Story of Breast Cancer Treatment and Recovery,&#8221; (Faber &amp; Faber, 1993).</p>
<p style="text-align: justify;">The second book examines the rarely discussed reality of metastatic breast cancer. In that book, &#8220;Advanced Breast Cancer, A Guide to Living With Metastatic Disease,&#8221; (O&#8217;Reilly, 1998), Mayer interviewed 35 breast cancer patients. Sadly, all of these patients had completed breast cancer therapy only to learn &#8212; often many years later &#8212; that the disease had returned. Mayer corresponded with many of these women through the Breast Cancer Listserv. Via the Internet, she met hundreds of &#8220;patients, family members and friends, researchers, nurses, doctors,&#8221; all focused on breast cancer and its treatment.</p>
<p style="text-align: justify;">In the 11 years since her diagnosis, Mayer has never experienced a recurrence of her cancer, but she said the potential for recurrence is a cloud that hangs heavily over the heads of any one ever treated for breast cancer. Metastatic disease, Mayer says, needs a larger forum in today&#8217;s discussions about the disease.</p>
<p style="text-align: justify;"><strong>Action Vs. Awareness</strong><br />
&#8220;It is not a pink ribbon disease,&#8221; she said of the generally positive message painted in the media about breast cancer and its prognosis. &#8220;The most honest use of the pink ribbon is the one in Ottawa. The ribbon is in the shape of a tear drop and the inside of the ribbon is lined in black.&#8221;</p>
<p style="text-align: justify;">&#8220;Dealing as I do with this forgotten population, women and some men who are coping with advanced breast cancer, I have discovered that these are the people everyone is afraid to hear from,&#8221; she said.</p>
<p style="text-align: justify;">Mayer started her career as a therapist and worked for many years as a Master&#8217;s level counselor in the Ohio Community Mental Health system, with a particular focus on women&#8217;s issues. But her enthusiasm for writing led her to complete a Masters of Fine Arts degree in writing at Columbia University in New York.</p>
<p style="text-align: justify;">She regularly teaches memoir writing, and leads writing workshops and retreats for people with life-threatening illnesses.</p>
<p style="text-align: justify;">&#8220;My focus has been on helping women once they receive a diagnosis, I believe I am helping them think about their treatment decisions and to think about that in a rational way,&#8221; she said.</p>

			<content:encoded><![CDATA[<p style="text-align: justify;">For a full year, Musa Mayer, a wife, mother, author and patient advocate, went about her life knowing she had a mass of substantial size in one breast. She knew the lesion was there because she could feel it. And it haunted her.</p>
<p style="text-align: justify;"><span id="more-387"></span>Mayer wasn&#8217;t martyring herself, or committing her life to being a victim. As soon as she discovered the lump, she went to her doctor to have it diagnosed and stumbled into one of the major flaws in breast cancer diagnosis. Because her doctors could not see the lump on a mammogram, Mayer was essentially given a clean bill of health. It took an interval of 12 months, strong self-advocacy and more extensive tests for the 3.5-centimeter tumor to be found.</p>
<p style="text-align: justify;">That experience 11 years ago has provided fuel for the brand of advocacy she practices for others today. She counsels newly diagnosed breast cancer patients through the medical maze, and inspires countless others through her books.</p>
<p style="text-align: justify;"><strong>A Need for New Diagnostic Tools</strong><br />
&#8220;I know that when Breast Cancer Awareness Month was initially started it provided a way for women to understand the importance of early detection,&#8221; Mayer said, referring to the emphasis on yearly mammograms. &#8220;But I think most women who have turned on their televisions or read a newspaper or magazine in recent years have already gotten that message and it&#8217;s time to move on.&#8221;</p>
<p style="text-align: justify;">Her own experience with mammography taught her that the low-dose diagnostic X-ray remains an imperfect tool. Mammography works best when used to diagnose the condition of post-menopausal women in whom fatty tissue has pervaded the breast. In premenopausal women, such as Mayer when she found her lump, dense glandular tissue can make a tumor difficult, if not impossible to see. Even some post-menopausal women on hormone replacement therapy, Mayer points out, may have denser tissue that can obscure a tumor. Glandular tissue appears white and opaque on a mammogram compared with the grayish color of fat. Tumors also appear white.</p>
<p style="text-align: justify;">What women need, Mayer says, are better diagnostic tools and more precise information about the state of breast cancer diagnosis. Mammography, she underscored, is incapable of pinpointing tumors in all women.</p>

<p>Previews of the Next Episode</p>
<p>The next episode is a &#8220;best of&#8221; episode. They claim that they show scenes &#8220;Never before seen&#8221; but I have yet to see one of these episodes with anything really new. For example, based on how the producers depicted Erin, the only reason she was selected was because of her beautilicious body. I&#8217;m sorry, Erin. Maybe you have a mind. Maybe you have substance. </p>

<p>Evaluating the program</p>
<p>There are a number of ways to evaluate the success of your boot camp program. Standard measures such as usage, retention and new sign ups are helpful, but membership feedback is the most powerful. </p>

			<content:encoded><![CDATA[<p style="text-align: justify;">Oral contraceptive pills (OCs) are the birth control method of choice for over 12 million American women. Used as directed, OCs are extremely effective at preventing unwanted pregnancies, and have an excellent safety record. When women fail to follow prescribed dosing instructions, for example by not taking a pill daily, the failure rate increases substantially. <span id="more-377"></span>A new study published in the May 9 issue of the Journal of the American Medical Association promises assistance for women who have problems with the daily dosing requirement of OCs.</p>
<p style="text-align: justify;">A team of researchers from the ORTHO EVRA/EVRA 004 Study Group presented data that compares traditional OCs with a skin patch that delivers similar hormones. The potential advantage of the patch is that it need only be applied once per week, and thus avoids problems with daily dosing requirements.</p>
<p style="text-align: justify;">Forty-five clinics in the United States and Canada provided the study participants—1,417 healthy women between the ages of 18 and 45 years who were sexually active and capable of becoming pregnant. The women were randomly assigned to receive either OCs on a typical daily dosing schedule, or the new skin patch. Each patch was worn for seven days at a time, for a total of 21 consecutive days per month; one week was patch-free.</p>
<p style="text-align: justify;">Effectiveness of the two modes of contraceptive therapy was evaluated by calculating an index of the number of pregnancies per 100 person-years of use, as well as by comparing actual with calculated probability of pregnancy. On both measures, the efficacy of the patch was slightly, (although not significantly) better than that of the OCs. Over all, the probability of pregnancy through 13 monthly cycles was 1.3 percent in the patch users, and 1.8 percent in OC users. This figure includes pregnancies resulting from both failure of the method employed, as well as those due to poor patient compliance with directions for use.</p>
<p style="text-align: justify;">Dr. Audet and colleagues reported that the types and frequencies of adverse events were similar in women using patches or pills for contraception. Women in both groups reported some breast discomfort (more in the group using the patch, but only for the first two monthly cycles). Other adverse effects included: headache; nausea; application site skin reactions for patch users; upper respiratory tract infections (17.9 percent in OC users vs. 13.3 percent in patch users); dysmenorrhea (painful periods) (9.6 percent of OC users vs. 13.3 percent of patch users); and abdominal pain (8.1 percent in patch users vs. 8.4 percent in OC users).</p>
<p style="text-align: justify;">Although initially higher in the patch users, by the third cycle on their respective contraceptives, both groups of women reported similar rates of breakthrough bleeding or spotting.</p>
<p style="text-align: justify;">Gilbert L. Ross, M.D., medical director of the American Council on Science and Health, concurred. He stated, “If the results of this study are confirmed by further research, women will have access to another convenient and reliable means of avoiding unwanted pregnancies.”</p>

			<content:encoded><![CDATA[<p>To encourage the mature market to participate in strength training, the Cooper Fitness Center organized a group strength-training program called CREST (Classics&#8217; Resistance Exercise Strength Training), which is designed to encourage independence and improve quality of life for participants. CREST concentrates on the stabilization muscles (neck, shoulders, back, abdomen and hips) to &#8220;improve core strength, thus improving balance, gait, postural alignment and general body awareness,&#8221; says Michael Kolling, mature market director. Measurable results are seen after completing the seven-week program.<span id="more-374"></span></p>
<p>Adults 55 and older are permitted to join CREST, with classes limited to eight participants who are instructed by certified personal trainers. Prior to joining, participants fill out a detailed medical history and are given personal fitness assessments to ensure proper programming for individual strengths and weaknesses. As part of the program&#8217;s educational curriculum, reviews and questions are encouraged and anatomical names and functions are taught.</p>
<p>Staff at the Cooper Fitness Center realized that the success of this type of program depends on two factors. First, the participants must feel comfortable in this type of exercise atmosphere. Since CREST exercisers know that they have to be 55 or older to join, knowing they will be exercising with peers gives them comfort. Participants do not feel incapable, intimidated or ineffective, but rather begin to feel a sense of independence.</p>

<p>Kolling sums up the CREST success: &#8220;Not only are the members becoming more fit and improving their quality of life, they are becoming more acclimated to the club setting and the benefits provided.&#8221;</p>

<p style="text-align: justify;">What enables such consumption is food importation, and foods are usually imported from developing regions, where surveillance may be inadequate in terms of food safety. <span id="more-363"></span>With favorable weather conditions and low labor costs, food importation will continue to increase.</p>
<p style="text-align: justify;">But through education and good management, the highest sanitation standards can be brought to bear to make imported produce equal in overall quality to domestically grown products. In the United States, employee education, good management and regulatory supervision are effective deterrents to behaviors that can lead to outbreaks of food poisoning. Most operators of chain restaurants and food service organizations &#8212; and even many operators of independent restaurants, which are often small &#8212; are aware of what must be done to prevent foodborne illness and act accordingly. But the application of additional strategies is desirable.</p>
<p style="text-align: justify;">Using microprocessors to control food preparation and handling processes in places where food is served has great potential for minimizing the possibility that human error will cause foodborne illness in customers.</p>
<p style="text-align: justify;">For example, a microprocessor-controlled freezing machine for dispensing soft ice cream was introduced commercially a few years ago. Properly sanitizing the previously available ice cream-dispensing machines requires disassembling them daily. Because this is tedious and usually scheduled for the end of the workday, to expect constant adequate diligence would be unreasonable. Microbiological trouble was an inevitable consequence.</p>
<p style="text-align: justify;">The microprocessor in the new dispensing machine makes daily disassembly unnecessary. The freezer component is programmed to enter a heating cycle at the end of each workday. Essentially, its contents are repasteurized, brought to a temperature slightly above freezing and kept at that temperature until the machine is reactivated. Manual cleaning, which requires disassembly, is necessary only after 14 cycles &#8212; the program&#8217;s limit. This fail-safe system almost eliminates the human element.</p>
<p style="text-align: justify;">Many experts consider instances of food poisoning reported to government officials just the tip of the iceberg. The incidence of illness due to improper preparation or handling of food at home &#8212; most of which goes unrecorded &#8212; has not gotten attention commensurate with the seriousness of the problem.</p>
<p style="text-align: justify;">There have been rumbles about high-tech companies foreseeing opportunities in controlling home kitchens. Considering such benefits as those of the microprocessor-controlled freezing machine, bringing home kitchens on-line would be a decidedly positive step toward improving food safety in the home.</p>
