			<content:encoded><![CDATA[<p>Your 14-year-old son has always been healthy. Last fall&#8217;s jacket is now too short, and you notice his voice is lower too. But he&#8217;s a head shorter than his best friend of the same age, and he&#8217;s wondering when he&#8217;s going to get tall. What can you and he expect?<span id="more-487"></span></p>
<p>The first sign that puberty is beginning for a boy is an enlargement of the testes and scrotum. Testes enlargement marks the increased production of testosterone and other male hormones (androgens) that bring about the other dramatic changes of male adolescence.</p>
<p>During elementary school, the average boy grows about 2 to 2.5 inches each year. In the year or two before puberty begins, height growth slows down a bit.</p>
<p>The average boy&#8217;s height growth spurt begins at age 12, usually about two years after girls begin their pubertal height increase. However, some boys may start growing as early as age 9 or 10, or as late as 14 or 15. The growth spurt lasts about four years.</p>
<p>A boy&#8217;s peak rate of growth usually happens at 13 or 14, but it can occur anywhere between ages 11 and 15. At his peak rate a boy is growing as much as 3.5 to 4.5 inches per year. No wonder he is constantly in the refrigerator! Boys generally reach their adult height by age 17 or 18, but we&#8217;ve all known young men who continued growing during the college years.</p>
<p>Testosterone also stimulates muscular development in the growing boy, as well as the bone growth that results in increased height. The adolescent weight spurt in the boy occurs at about the same time as the height spurt.</p>
<p>Your son will gain 12 to 25 pounds a year (on average, about 20 pounds a year) during the height spurt. A good deal of that weight is muscle and bone. A boy&#8217;s weight increases from about 85 pounds at the beginning of puberty to an adult weight of 150 pounds or more by age 18.</p>
<p>Androgens are also responsible for other changes in the genitals of boys. The penis grows in length and then later in width, usually about a year before the boy reaches his peak growth rate. The developing testes produce sperm and semen relatively early in adolescence. The boy&#8217;s first ejaculation of sperm usually occurs at age 13 or 14, often in the form of a &#8220;wet dream&#8221; or during masturbation.</p>
<p>Pubic and underarm hair develops soon after genital growth, along with the maturation of skin glands. Sweat glands produce &#8220;adult&#8221; sweat with odor, and oil glands on the face can become clogged with oily material and develop into acne, especially around age 15 or 16. Boys may be disturbed about the acne but reluctant to request help in managing it. New medications can be a big help.</p>
<p>Facial hair usually appears about age 15, first on the upper lip, then later on the chin and in the sideburn area. In general, chest and other body hair do not develop until late adolescence or early adulthood.</p>
<p>Testosterone increases the size of the boy&#8217;s larynx (voice box), which becomes more visible as the &#8220;Adam&#8217;s apple.&#8221; The thickening of the vocal cords results in the lowering of the adolescent&#8217;s speaking and singing voice &#8212; sometimes rather unexpectedly!</p>
<p>All those inches and pounds of growth are associated with an increase in the size of the brain, liver and other organs, and an increase in the number of red blood cells &#8212; all of which mean the boy needs a diet rich in protein, calcium, iron and other nutrients for optimal growth.</p>

<p>Your boy&#8217;s growth pattern or tempo may not be the same as his friends&#8217; &#8212; he may grow &#8220;too soon&#8221; or &#8220;too late&#8221; in his mind. Compared with girls, adolescent boys are less likely to have visits with their pediatrician. Your son&#8217;s doctor can make the most of school sports and camp physical visits by assessing his growth and reassuring him about normality as well as giving other good-sense adolescent advice.</p>

<p style="text-align: justify;">So who do you call? The internet of course. If it exists, you will either find it, or a lead on the web. For instance, did you know that most major sewing pattern companies have a website? Did you know that you can locate the closest fabric chain near you by using their website? Did you know there are thousands of free instruction sheets on everything from constructing bird houses to making rag dolls?</p>
<p style="text-align: justify;">FINDING it of course is the real challenge. It all starts at a search engine. A search engine is simply a website that uses key words that you enter to search on for more information. There are lots of good search engines out there; they make their money by advertising products and other websites, often driven by what kind of search you are doing. But they are free for you to use. <p style="text-align: justify;">The more specific your key words, the more specific your information. Let&#8217;s say that you want to find some instructions for making hand made dolls and doll clothes. Entering the word DOLL at the search engine will get you many more times as many &#8220;hits&#8221; or websites that the word doll appears on than entering &#8220;doll&#8221; and &#8220;instructions&#8221;. So when you are entering information, narrow your search as much as possible. Entering the word &#8220;free&#8221; will narrow the field even more; now you are eliminating all the commercial websites.</p>
<p style="text-align: justify;">Now that you have your criteria entered, you will hopefully have some quality &#8220;hits&#8221; (results). The only way to tell this is to browse the descriptions, and sometimes to actually visit the sites. This can be time consuming, and you learn as you go what things are valuable. Don&#8217;t be afraid to make a judgement call; the information superhighway is fabulous, but not every site is going to take you where you want to go. And its usually worth the effort!</p>

			<content:encoded><![CDATA[<p>The start of a new year is a double-edged piece of military cutlery &#8212; a psychological new beginning &#8212; a fresh start. It also reminds us of last year when we declared war against our fat &#8212; again. Viva la resolution!<br />
I gave up making New Year&#8217;s resolutions several years ago. My list seldom grew beyond the perennial entries of losing weight and getting off my big round rosy butt to exercise. Ah, tradition.<span id="more-479"></span></p>
<p>Resolutions that start out sensibly enough all too often grow unwieldy and unrealistic. They are line items on an emotionally charged, yearlong &#8220;to-do&#8221; list. We frame them in negative terms that involve losing, cutting back, giving up, and not doing this or eating that. Resolutions that, in the end, may limit instead of liberate.</p>
<p>Before the year has had a chance to stretch its legs, widespread self-flagellation (how many calories does that burn?) among rotund resolution makers runs rampant. We feel frantic if we haven&#8217;t lost all excess weight within the first few weeks of the year &#8212; no matter what the amount. What will people think if we&#8217;re still fat after a month&#8217;s time?! Consequently, we may stall our progress or even gain additional pounds.</p>
<p>One challenge this yearly declaration created in my life included public scrutiny. In the past if I let slip my intention to refocus efforts toward the reduction of my voluptuous visage, well-meaning friends, family and colleagues would barrage me with &#8220;supportive&#8221; inquiries about how I was doing long before the first week of the New Year had a chance to cool.</p>
<p>Knowing that someone, anyone, was keeping track of my efforts (even casually) made me feel unsettled and anxious. The fear of public failure and the anticipated resultant humiliation was paralyzing. My health and my welfare took a backseat to my inordinate desire to please others and to earn their approval. I craved their praise and acceptance, but the fear of being a sorry disappointment to them made me run to the fridge for something to eat to sooth my frazzled psyche. I knew comestible consumption was counterproductive to my desired outcome, but I felt helpless to stop.</p>

It is not unusual for fat folks to forgo their emotional and spiritual needs in deference to the desires of others. </a></p>
<p>Once that shift occurs most success we do achieve is limited or short-lived.</p>
<p>How do we assuage this annual self-inflicted assault? We have to look at what we really want out of our resolution to lose weight. Although improving our health and vitality is certainly of prime importance, the deeper issue is usually acceptance.</p>

			<content:encoded><![CDATA[<p>Many colleges are also recommending that students receive immunization against the bacterium Neisseria meningitidis, or meningococcus, which causes very severe and often fatal meningitis (brain infection) among college students and other young people who live together (in college dorms and in Army barracks, for example). The meningococcal vaccine does not provide protection against every form of the meningococcus bacterium, but it is still recommended.<span id="more-476"></span></p>
<p>All entering students in a boarding college must also demonstrate that they are free of tuberculosis. The pediatrician will give your student a TB test, or Mantoux test (PPD) on the arm. The test must be checked 48 hours after it is put on the arm, and the pediatrician&#8217;s office must verify that the test was negative for TB. If the test is positive, your student will have a chest X-ray and will begin an anti-tuberculosis medication for several months.</p>
<p>But there is so much more to the pre-college visit than filling out complicated health forms and giving shots, as important as those are. The doctor will use this visit to discuss and review with your student important health issues such as exercise and fitness , including developing an &#8220;adult&#8221; exercise program such as working out, golf, tennis and jogging when team sports aren&#8217;t available eating right to maintain a normal weight and avoid adult cardiovascular problem savoiding the use of tobacco, alcohol, and other substances, and managing alcohol intake at college<br />
sexual health, including protection from pregnancy and sexually transmitted diseases (HIV, herpes, gonorrhea, chlamydia and others)<br />
campus safety, including avoiding violence, &#8220;date rape&#8221; and other problems associated with intoxication<br />
managing homesickness, stress, eating disorders, depression or suicidal feeling.</p>

			<content:encoded><![CDATA[<p>One of the most important visits to the pediatrician is also one of the last few: the college physical. The pre-college visit &#8220;launches&#8221; your high school graduate toward adulthood, and you can be sure your pediatrician looks as proudly on your young adult child as you do!<span id="more-473"></span></p>
<p>The pre-college visit is a terrific opportunity for your young person to talk with the pediatrician about a whole variety of health behaviors and issues. And it often marks the beginning of your adolescent taking on the adult responsibility and decision-making about her own healthcare.</p>
<p>The college health service at your son&#8217;s or daughter&#8217;s college will usually require that a health form and physical examination be completed before college entry &#8212; but even if your child&#8217;s college doesn&#8217;t send a form, be sure to make an appointment with the pediatrician during the summer before college begins for this important health overview.</p>
<p>The college health form usually includes a fairly extensive questionnaire and health history that the student completes (she may need your help with some of the details about her past history) and that the pediatrician reviews. Because the college health service will be acting as your child&#8217;s health provider when she is away at college, it is very important that the student and the pediatrician provide as complete and as accurate information as possible.</p>
<p>The college health service needs to know what medications the student uses regularly, what allergies or bad reactions have occurred with medication use, whether your student has a chronic illness (such as seizure disorder, diabetes, asthma or muscular dystrophy), whether your child has had any surgery in the past and why, and many other questions.</p>
<p>The form also includes a physical exam, which is essential for students considering intramural or varsity sports &#8212; but important for all young people as the first &#8220;adult&#8221; examination. The physical exam allows the pediatrician to assess whether growth is complete &#8212; many boys continue to grow in height in the first years of college &#8212; and whether nutrition seems adequate. Many college forms encourage a blood count (hemoglobin or hematocrit), to be sure there is no anemia, and a cholesterol screen, which is often the first time cholesterol is screened.</p>
<p>Colleges also require evidence that the entering student&#8217;s immunizations are complete. Tetanus should be received about every 10 years (usually at age 15 and then 25), and the polio series should have been completed by age 5. </p>
<p><a href="http://pheromones-one.com/faq.php">Dates of two immunizations against measles, or proof of immunity to measles (a blood test) are required; the vaccine is usually given as MMR, or measles-mumps-rubella, after age 1 and after age 5. </a></p>
<p>Hepatitis B immunization is not required but is very strongly recommended. If your student has not had chickenpox (varicella), the varicella immunization is also very strongly recommended.</p>

			<content:encoded><![CDATA[<p style="text-align: justify;">At the time the first paper on the night eating syndrome was published, in 1955, there was no concept of eating disorders and such a concept did not arise for more than 25 years. <span id="more-456"></span>Before that, diagnosis was poorly regarded in the field of psychiatry, particularly by the psychoanalysts, as exemplified by Karl Menninger whose 1962 book, “The Vital Balance,” proposed that all diagnosis in psychiatry be done away with.</p>
<p style="text-align: justify;">With the waning influence of psychoanalysis, interest in diagnosis increased, an increase that can be measured by the mounting number of disorders in the successive editions of the Diagnostic and Statistical Manual (DSM) of the American Psychiatric Association. It was the publication of Russell’s 1979 paper on “bulimia nervosa” that provided the impetus for the designation of eating disorders as a special group (Psychological Medicine 1979; 9:429–448). This disorder, the cardinal features of which were vomiting following a binge, in order to prevent weight gain, was introduced with the proposition that it was very resistant to treatment. The importance of diagnosis is illustrated by the fact that, within two years of the publication of Russell’s paper, there were three successful reports of the treatment of bulimia nervosa. Subsequent years have seen an explosion of research on treatment, which has become increasingly effective. Russell’s report awakened interest in eating disorders but none that involved obesity. For many years, eating disorders meant anorexia nervosa and bulimia nervosa.</p>
<p style="text-align: justify;">In recent years “binge eating disorder” has been proposed to describe persons who binge but do not engage in compensatory behaviors, such as vomiting, to control their body weight. This disorder has not been accepted as a diagnosis in the Diagnostic and Statistical Manual.</p>
<p style="text-align: justify;">During this time progress was made in our understanding of the night eating syndrome, although it, too, has not been accepted into the Diagnostic and Statistical Manual. Most studies of the night eating syndrome have attempted to determine the prevalence of the disorder, which, as noted above, is far more common than the recognized eating disorders. Recruiting for a study of binge eating disorder allowed us to determine for the first time the relative frequency of the two disorders (binge eating disorder and the night eating syndrome) (Stunkard et al, International Journal of Obesity, 1996; 20:1–6). In this sample, self selected as it was for binge eating disorder, this disorder was somewhat more prevalent (20 percent) than was the night eating syndrome (14 percent).</p>
